MyCareAnyWare - Simplifying Chronic Disease Management

Chronic Care Model Helps Patients to Take Control of Their Health

AnyWare Group’s MyCareAnyWare, the chronic disease mangement solution, is built on their industry leading secure remote access technology infrastructure, Role Oriented Access Management (ROAM). By using ROAM, MyCareAnyWare enables patients to securely access select medical files and disease education resources via an Internet connection, also update their health status, and allowing core hospital teams analyze this information, thus simplifying chronic disease management.

MyCareAnyWare is an effective approach to patient self management that helps maintain independence and improve health through prevention and management of chronic conditions.

Newly diagnosed diabetes patients will be able to access the Chronic Disease Management (MyCareAnyWare ) system anytime, anywhere and enter crucial details such as their blood sugar levels, blood pressure and heart rate, to their medical files rather than visiting diabetes clinics with long wait times. Allied health specialists will monitor each patient's ongoing status and when imminent problems are identified, an alert is sent to the patient's physician immediately.

ROAM supports MyCareAnyWare by facilitating the decline of wait lists and alleviating pressure on congested health care facilities. This chronic care model helps eliminating overcrowded clinics, patient care is enhanced and wait times are reduced.
